In a heartwarming celebration filled with joy and gratitude, twin brothers Amari and Javar Ruffin recently marked their first birthday. This milestone comes just a month after their successful separation surgery at the Children’s Hospital of Philadelphia (CHOP), where a dedicated team of surgeons gave the boys the chance to live independent lives after nearly a year of being connected.
The brothers, who were joined at the sternum, diaphragm, abdominal wall, and liver, spent the first year of their lives overcoming unimaginable challenges. For their parents, Tim and Shaneka Ruffin, this milestone is nothing short of a miracle. “The whole situation was a crazy journey, but to be here right now is still surreal to this day. It’s beautiful,” Tim said in an interview with Good Morning America.
A Rare and Complex Condition
Amari and Javar were born as omphalopagus twins, a rare condition where twins are connected at the front of the body and often share vital organs. Conjoined twins are an exceptionally rare phenomenon, occurring in approximately one out of every 35,000 to 80,000 births. When a routine ultrasound 12 weeks into Shaneka's pregnancy revealed the twins' condition, doctors initially advised terminating the pregnancy due to the complexity and risks associated with conjoined twins.

“We were home when we actually got the phone call from the specialist telling us all the details about the twins,” Shaneka said. “When she said that it was best that we terminate, it was a lot of emotions, because that was the first thing that was said.” However, Tim and Shaneka decided to seek a second opinion, turning to CHOP, one of the few hospitals in the world with expertise in separating conjoined twins. After receiving encouraging news from the experts at CHOP, the Ruffins decided to move forward with the pregnancy, holding onto hope that their boys could be separated.
A Year of Careful Planning and Preparation
After Amari and Javar were born via cesarean section on September 29, 2023, weighing about six pounds collectively, they spent 10 months at CHOP, where a dedicated medical team meticulously prepared for their separation surgery. The boys underwent numerous tests, including MRIs and echocardiograms, which provided a detailed map of their shared anatomy. Their team of specialists, led by Dr. Holly L. Hedrick, rehearsed the complex surgery for months, carefully coordinating every step to ensure the best possible outcome.

“Separating conjoined twins always presents challenges because each set of twins is different,” said Dr. Hedrick, a pediatric general and fetal surgeon at CHOP. “We spend hours rehearsing the flow of surgery prior to the big day – even practicing how to transfer each baby to their own bed. We work collaboratively as a team, ready to combine our unique skills to achieve a safe separation and a chance for these babies to lead healthy and fulfilling lives.”

A crucial aspect of the surgery was that Amari and Javar each had their own fully functional hearts. “These hearts being separate from each other, and being completely functionally and anatomically and structurally normal was a huge win,” Dr. Hedrick explained. “I'd say 90% of conjoined twins that are diagnosed in the world don't make it to this point that the boys have made it to.” However, the full complexity of the surgery would not be known until the team began the procedure.

The Life-Changing Surgery
On August 21, 2024, after months of careful planning and preparation, a team of over two dozen specialists, including surgeons, anesthesiologists, radiologists, and nurses, gathered for the eight-hour surgery that would change the lives of Amari and Javar. The surgeons carefully separated the boys, dividing their liver and rebuilding their abdomens using mesh and advanced plastic surgery techniques. The team celebrated a huge victory as the twins were successfully separated, each placed into their own bed for the first time since birth.

Dr. Hedrick explained the emotions behind such a complex procedure, stating, “It's very nerve-racking in the beginning just to try to nail the diagnosis, just to try to be sure that we completely understand the anatomy, because a lot of times, it's an impossible feat.”

The boys' recovery journey has been nothing short of remarkable. According to their parents, both Amari and Javar are now beginning to establish their independence, learning to sit up, roll, scoot, and crawl. “Javar is spicy. Amari is more laid-back,” Shaneka said, describing how her sons' personalities are starting to shine.
A Grateful Family and a Bright Future
For the Ruffin family, the separation surgery marked the beginning of a new chapter in their lives. After nearly a year in the hospital, the family was finally able to return home on October 8, 2024, where Amari and Javar reunited with their older siblings, Kaylum and Anora. The boys continue to receive physical and speech therapy as they work toward achieving their developmental milestones.

“Seeing them each in their own beds was an indescribable feeling,” Shaneka said. “It feels like we are beginning a new journey as a family of six. We are so grateful to CHOP for helping make this day possible and letting us start this next chapter.”
The Ruffins' journey, from the initial diagnosis to the successful surgery, is a testament to their determination and the exceptional care they received at CHOP. Reflecting on their experience, Tim encourages others facing similar challenges to never give up. “Don’t give up,” Tim said. “This is a hard journey, but don’t give up.”
